Service Coordination
Service Coordination is focused on achieving results. For those in nursing homes, the aim is a safe transition back into community living. For those already in the community, the objective is sustained community living as preferred. Support is provided to access essential services.
Available services include:
- Service Coordination (SC)
- Independent Living Skills Training Programs (ILST)
- Positive Behavioral Intervention Supports (PBIS)
- Community Integration Counseling (CIC)
- Home and Community Support Services (HCSS)
